Each school may nominate up to five (5) students who meet the eligibility requirements for the REACH Youth Scholarship. Please provide the names and contact information for your nominees below. ***Only students listed on this form will be considered for the scholarship.***

REACH Youth Scholarship Criteria
- Nominees must be seniors graduating from a Santa Clara County high school at the end of the 2024-2025 academic year with a cumulative GPA of 2.50 or higher.
- Nominees must participate in at least one sanctioned high school athletic activity.
- Nominees must have demonstrated the ability to overcome adversity/obstacles to reach their goals.
