Partnering with Morling College in the formation of students
Thank you for your interest in participating in this learning activity. Please read the following information about the activity, so that you can decide whether you would like to take part. Please feel free to ask any questions you might have about your involvement.

This learning activity provides an opportunity for students to reflect on their learning through guided conversations with a Formation Mentor. The goal is to help students process their understanding of themselves, their world, and Christian ministry.

I consent to participate as a Formation Mentor for the student below.

I confirm that I have received and read the Information for Formation Mentors document.

I understand that:
- The purpose of this activity is to help students reflect on their learning and its implications for their faith, personal growth, and ministry.
- I will meet with the student three times during the study period (typically Weeks 4, 8, and 11 of a standard semester, or Weeks 3, 6, and 9 in summer online study).
- If the student is enrolled in multiple units requiring a mentor, they still only need three meetings total.
- Our conversations will be confidential, unless the student gives permission to share, or I am legally required to disclose information.
- This activity is for training and formation only—any notes or reflections are for the student's use and will not be published.
I also acknowledge that:
- My participation is voluntary, and I can withdraw at any time without penalty or bias.
- If I withdraw, the student must remove any references to our conversations from their reflection.
- Any information shared in the student’s reflection will remain confidential, accessible only to the student, Morling staff, and the AUT moderator.
  My personal details (e.g., phone number, email) will not be shared outside Morling College.
- I have received contact details for the student, their supervising academic staff member, and the AUT Ethics Committee.
