ENROLLMENT: This registration form must be completed and submitted to It's Music Time with the appropriate payment information, signatures, and agreements prior to the first class. A debit or credit card must be on file at IMT unless specific arrangements have been approved by IMT prior to enrollment. Your credit card will be run at the first (1st) of every month until notification of cancellation is received by IMT.* This field is required.

ATTENDANCE: Tuition paid to It's Music Time includes one music class per week except when IMT is closed for holidays. A calendar will be emailed to you along with confirmation of payment and enrollment. Class times are reserved for your enrolled child.* This field is required.

CANCELLATION: You may cancel your child's enrollment in IMT at least five (5) days prior to the first (1st) of the month. Cancellations which have not been made five (5) days before the first (1st) of the month will be automatically charged and your child will be re-enrolled for classes that month.* This field is required.

REFUNDS: Tuition paid reserves a spot in a class at a specific time and no refunds or credits shall be issued for missed classes. If an extended illness requires you to miss two or more classes, we will offer a one class credit toward the next month's tuition.* This field is required.
