Acknowledgements
Acknowledgment of Risks: I, the undersigned, acknowledge that participation in Sweetwater Kids Summer Day Camp involves certain inherent risks, including but not limited to physical activity, outdoor activities, and other camp-related events. I understand that accidents, injuries, or illnesses may occur, and that these risks are part of the activities in which my child will be involved.

Assumption of Risk: I hereby voluntarily and knowingly assume all risks associated with my child’s participation in the Sweetwater Kids Summer Day Camp, including but not limited to personal injury, damage to property, and any other incidental or consequential damages resulting from camp activities. I acknowledge that these risks cannot be entirely eliminated and that the Camp cannot guarantee absolute safety.

Release of Liability: I, on behalf of myself, my child, and any of my heirs, executors, administrators, or assigns, do hereby release, waive, discharge, and hold harmless Sweetwater Kids Summer Day Camp, its owners, staff, volunteers, affiliates, and agents from any and all liability for any injuries, damages, or losses (including those caused by negligence) sustained by my child during participation in the Camp's activities.

Medical Treatment Authorization: In the event of illness, injury, or medical emergency during my child’s participation in the camp, I hereby give permission to the Camp to seek medical treatment for my child, and I agree to be responsible for all costs related to medical treatment, including but not limited to hospital bills, ambulance fees, and physician charges.

Conduct Expectations: I agree to ensure that my child follows the rules and guidelines set forth by the Camp staff and that my child’s conduct will not endanger the safety or well-being of themselves or others. I understand that failure to adhere to the Camp’s rules may result in removal from the program, without refund.
