The National Child and Youth Disability Reimbursement Program provides up to $3,000 per child per year to Regular and Reserve Force CAF members with dependent school-aged children with disabilities including neurodevelopmental, social, physical, and medical needs who require accommodations to access their educational curriculum.

Receipts are not required until this form is approved.
· This form is to determine your eligibility for reimbursement of assessments, therapies and/or equipment costs incurred from 01 April 2025 until program funding has been exhausted.
· A separate Eligibility Confirmation Form is required for each dependent child.
Eligible Categories:
Assessments and/or Therapies: This includes but is not limited to: services such as Applied Behavioral Analysis/Intensive Behavioural Intervention, speech, physical and occupational therapies, and psychological and educational assessments not already covered by insurance.
Assistive Devices and/or Equipment: This includes but is not limited to: sensory equipment, specialized devices to support access to the curriculum such as an FM system; specialized mobility aids; specialized seating; technology including a laptop, applications, specialized reading tools, specialized writing tools as identified in a student Individual Education Plan (IEP) or equivalent validation from the applicant’s school or professional assessment, not already covered by insurance.
