By submitting a nomination through this form, you acknowledge that you have read, understood, and agreed to the following Terms & Conditions governing participation in the Heroes Among Us – A Surprise Thank You initiative.

1. PURPOSE & SCOPE

The Heroes Among Us initiative is designed to recognize and reward first responders, military personnel, and medical providers through spontaneous surprise moments in their everyday lives. These surprise interactions will be recorded and publicly shared via FundTheFirst.com’s social media channels and other platforms.

Through this nomination form, individuals can nominate a deserving hero and provide details for consideration. However, submitting a nomination does not guarantee that the nominee will be selected.

2. NOMINATION REQUIREMENTS

To submit a nomination, you must provide the following information accurately and completely:

A. About You (The Nominator)

   •    Full Name

   •    Email Address (for contact and coordination purposes)

   •    Phone Number

   •    Relationship to the Nominee (e.g., friend, colleague, family member, community member)

B. About the Nominee (The Person You Are Nominating)

   •    Full Name

   •    Occupation (Must be a First Responder, Military Member, or Medical Provider)

   •    City & State of Residence

   •    Brief Story on Why They Deserve Recognition (Be as detailed as possible; this helps with selection.)

C. Coordination Details

   •    Best Time & Location to Surprise the Nominee (While Off-Duty) (e.g., gym, grocery store, coffee shop, community event, etc.)

   •    Your Availability to Assist with Coordination (We may need your help in setting up the moment.)

3. SELECTION PROCESS

   •    FundTheFirst.com anticipates receiving thousands of nominations nationwide. While we will carefully review each submission, only a limited number of nominees will be selected due to time, funding, and logistical constraints.

   •    Submissions will be reviewed on a rolling basis, and selection is at the sole discretion of FundTheFirst.com and its team.

   •    Selection criteria may include: compelling stories, impact on the community, and logistical feasibility of executing the surprise.

4. USE OF DONATIONS

   •    100% of all funds raised through this initiative will be used solely to provide rewards to the selected nominees.

   •    Funds from this campaign will NOT be redirected, reallocated, or used for any purpose other than recognizing and surprising nominated heroes.

   •    Any tips given through the fundraiser will also be reinvested directly into funding more surprise recognitions.

5. CONSENT & RELEASE AGREEMENT

By submitting a nomination, you expressly agree to the following:

   1.    Filming & Public Sharing: If your nominee is selected, you acknowledge and consent to the surprise interaction being filmed, photographed, and recorded. These materials will be shared publicly on FundTheFirst.com’s social media platforms and other media outlets.

   2.    Permission for Contact: You agree that FundTheFirst.com may contact you via email or phone to coordinate logistics for the surprise.

   3.    No Expectation of Compensation: You acknowledge that neither the nominee nor the nominator will receive any monetary compensation, beyond what is awarded to the nominee through the surprise gift.

   4.    Privacy & Confidentiality: Personal information collected through this nomination form will be used exclusively for this initiative and will not be shared with third parties except as necessary for execution.

   5.    Accuracy of Information: You confirm that all information provided is accurate, truthful, and does not misrepresent any facts. Submitting false or misleading information may result in disqualification from participation.

   6.    Legal Liability: You acknowledge that FundTheFirst.com, its affiliates, and partners are not responsible for any damages, losses, or claims arising from the selection process, participation, or non-selection of a nominee.

6. RIGHT TO MODIFY OR TERMINATE

   •    FundTheFirst.com reserves the absolute right to modify, suspend, or discontinue this initiative at any time, without prior notice, based on funding availability, logistical feasibility, or other factors.

   •    Selection of nominees is subject to change at FundTheFirst.com’s sole discretion.

7. ACCEPTANCE OF TERMS

By submitting this nomination, you confirm your understanding and acceptance of these terms. You also affirm that:

✅ You have obtained no confidential or protected information in making this submission.

✅ You understand that FundTheFirst.com is not obligated to select any specific nomination, and participation is voluntary.
