PURPOSE OF AGREEMENT
This Service Agreement outlines the support and services to be provided by EFOODZ t/a Mealcare to the Participant under the National Disability Insurance Scheme (NDIS). It ensures that the Participant’s meal requirements are met in accordance with their NDIS Plan.
SERVICES PROVIDED
EFOODZ t/a Mealcare agrees to provide the following services:
- Preparation and delivery of nutritionally balanced meals.
- Consideration of dietary requirements and preferences as specified by the Participant.
- Delivery of meals in accordance with the agreed schedule.
RESPONSIBILITIES OF EFOODZ t/a MEALCARE
EFOODZ t/a Mealcare agrees to:
- Provide high-quality meals that meet food safety standards.
- Deliver meals as per the agreed schedule.
- Communicate any changes to the service promptly.
- Ensure invoices are issued in accordance with NDIS guidelines.
RESPONSIBILITIES OF THE PARTICIPANT
The Participant agrees to:
- Provide accurate and up-to-date information regarding dietary needs.
- Notify EFOODZ t/a Mealcare of any changes to their NDIS plan or funding.
- Ensure timely payment for services provided.
- Inform EFOODZ t/a Mealcare of any concerns regarding the service.
PAYMENT AND PRICING
Services will be charged in accordance with NDIS Price Guide rates.
Payment may be made via:
- Self-Managed: The Participant pays directly.
- Plan-Managed: The Plan Manager will be invoiced.
- NDIA-Managed: EFOODZ t/a Mealcare will claim payments directly from the NDIS.
CANCELLATION POLICY
Participants must provide at least 1 week notice for cancellations.

Complaints will be handled promptly and in accordance with NDIS guidelines.
PRIVACY AND CONFIDENTIALITY
All personal and health information will be handled in accordance with privacy laws.
EFOODZ t/a Mealcare will not disclose personal information without consent.
