Request Interview to become an Admitted Candidate

By requesting to become an admitted candidate, you are asking to interview with your dCOM to be admitted into the candidacy process as the first step toward becoming licensed for ministry or ordination.
Complete the following form to request an interview with your dCOM. This form will be sent directly to your dCOM chair. Please notify your dCOM chair by email to notify him or her that your form is forthcoming.

To complete this form you will need:
1. A report from your pastor certifying that you have met with them and discussed The Christian as Minister or Blessed Wrestling. This can be a Word document.
2. Your call statement.
3. Biographical form 102, found online here: https://vaumc.org/candidates-for-ministry/
4. Candidate's Disclosure Form
