REQUEST FOR TRANSCRIPT OF ACADEMIC RECORD

INSTRUCTIONS & INFORMATION

• Please complete and submit this form with payment to request an official or unofficial transcript.
• Transcript requests are typically processed within 5 business days.
• The cost of official transcripts (hardcopy or digital) is $35.00 per copy
• Transcripts can be requested to be sent to institutions/organizations or can be issued directly to students.
• If a transcript is issued directly to a student, the transcript and envelope will be marked “ISSUED TO STUDENT”
• All hardcopy requests are sent via standard Canada Post.
• For expedited delivery you may select the option to send via courier at an additional fee. See below in order options.
• You must indicate this in your request if you desire expedited delivery.

ATTENTION

If you have selected a hard copy of your transcript to be sent standard mail or courier, you MUST complete all fields below for sending.  
IMPORTANT: P.O. boxes are not accepted for couriers

Please note:

• If NO shipping option is selected the default will be 'Standard'. 
• An Acupuncture Hours Letter may accompany transcripts requested for the CCHPBC, if applicable.
