Tucson Girls Chorus Mission

The mission of the Tucson Girls Chorus is to prepare a diverse group of young women to be confident leaders through participation in a well-constructed educational program that emphasizes music, performance, leadership training and community service.

General Model Release

I hereby give permission to the Tucson Girls Chorus Association, its agents and/or assignees, permission to use photograph(s) taken of the minor child named below for the Tucson Girls Chorus spring/fall concert programs and/or for other Tucson Girls Chorus promotional use. I relinquish all rights, title, and interest I may have in the finished pictures, negatives, and copies for this purpose. I waive the right of prior approval and hereby release the Tucson Girls Chorus from any and all claims for damages of any and all kinds based on this use of said material. I am of legal age, have the right to contract for the minor, and freely sign this release, which I have read and understand.

Emergency Treatment Release

In case of an emergency during any Tucson Girls Chorus activity in which she is participating, the leader(s) of the activity have my permission to seek and obtain medical attention from a licensed physician and to be admitted to the nearest hospital, if necessary.

Demographic Information and Annual Household Income

Demographic information is requested by Tucson Girls Chorus in order to provide information for Grant purposes.
You are not required to furnish this information, but are encouraged to do so. All information will remain confidential.

PAYMENT INFORMATION 

Tuition scholarships are available and we accept ESA funds. Please contact the TGC staff if you plan to use ESA funds so we can help provide the necessary documentation.

2024-2025 Scholarship Application (will be sent in your confirmation email after submitting this form).
