If you would like to become a member of The Elevate Fund Giving Circle, please complete the membership form below. Note: an annual contribution is required each year a member would like to participate. In your confirmation email you will receive a link to make a donation to the fund, commensurate to your membership giving tier. Thank you, and welcome to the Elevate Fund Giving Circle! Membership contributions can also be made as monthly recurring gifts! Please enter the amount you would like to give annually above. When you enter your donation at the link emailed to you at confirmation, you can choose to divide your gift into monthly installments. Ex.: $42/month equals a $500 membership or $84/month equals a $1,000 membership.
