Acknowledgment of Risks:
I, the undersigned, acknowledge and agree to the following:
I understand that entering, working in, or exiting a compost trailer involves potential risks, including but not limited to:
- Exposure to sharp objects, slippery surfaces, or unstable footing.
- Contact with organic materials that may cause allergic reactions, respiratory irritation, or other health concerns.
- Risks of injury from tools, equipment, or machinery in use around the trailer.
- I understand that it is my responsibility to follow all safety instructions, wear appropriate personal protective equipment (PPE), and exercise caution while in or near the compost trailer.
Assumption of Risk:
I voluntarily assume all risks associated with entering, working in, or exiting the compost trailer. I accept full responsibility for any injury, illness, or damage that may occur as a result of my participation.
Release of Liability:
In consideration of being granted access to the compost trailer, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release and discharge Fallen Oak Mycology, its employees, agents, and representatives from any and all claims, liabilities, demands, or causes of action arising from or related to my participation.
Indemnification:
I agree to indemnify and hold harmless Fallen Oak Mycology, its employees, agents, and representatives from any and all claims, liabilities, demands, or expenses (including attorney fees) resulting from my actions while in or around the compost trailer.
Agreement to Comply with Rules:
I agree to follow all safety protocols, signage, and instructions provided by Fallen Oak Mycology. I acknowledge that failure to do so may result in the termination of my access to the compost trailer.
Certification:
I certify that I am 18 years of age or older and that I have read, understood, and agree to the terms of this waiver. If I am under 18, I understand that a parent or legal guardian must also sign this waiver on my behalf.
