Hoover Intergenerational Care, Inc.
FRAUDULENT INFORMATION OR FAILURE TO REPORT CHANGES AFFECTING INCOME
Enrollment is based on eligibility according to income. If we find that eligibility for childcare is based on false information (incorrect income, marital status, number in household, etc), we are required to recover from you the entire cost of the child care services for the entire time of enrollment. In addition, if changes affecting eligibility are not reported, you will be charged retroactively for any changes in fees. The daily full cost fee for your service would be $62.66
In order to remain in compliance with state regulations, HIC must be notified within 30 days of any changes in your family affecting eligibility for services.
A list of changes of which H.I.C. must be notified is as follows: Change in income (if it exceeds 85% of SMI)
The State has recommended that, if a parent refuses to pay retroactive fees, we use collection agencies or small claims courts when necessary. Please be sure that your information is correct so we can avoid any problems.
I declare under penalty of perjury that this information is true and correct. I understand that I may be prosecuted for fraud and required to pay any money resulting from false or incorrect information provided herein. Legal action can result in penalties of a fine, imprisonment or both.
