e3Business Consulting Addendum

This Agreement is entered into as of:

and e3Business, LLC ("e3Business") and is bound by the same general terms as the e3Business Consulting Agreement previously executed between the parties.

1. Client shall pay e3Business fees of Practice Review is $500 for a practice evaluation review for Client to complete, a first follow-up discussion (up to 1 hour), a report assessing the practice, a second follow-up discussion (up to 1 hour) and 30-day access to e3FBM Premium Content, (terms of service and terms of use apply).
2. Client has 30 days to complete the Practice Review as follows:
   1. Client shall complete the practice questionnaire and return to or discuss with e3Business within five days of executing payment.
   2. Client shall schedule and complete the first follow-up discussion within five days after notice from e3Business that the evaluation of the practice evaluation has been completed.
   3. Client shall schedule and complete the second follow-up discussion within five days after receiving e3Business’ report following the first follow-up discussion.
3. Notwithstanding exceptional circumstances, e3Business reserves the right to stop the Practice Review without refund of fee if Client does not perform duties consistent with consistent with the above or fails to respond to e3Business’ requests necessary to complete the Practice Review.
