Finley Fort Foundation (FFF) can not guarantee full medical cost reimbursement. Not all applications will be approved. If approved, FFF will pledge up to $500 for the memorialization of the pet and pay the hospital or clinic directly when the treatment occurs. Memorialization includes: cremation, paw print, urn, and other related services. Please include additional items in the request or in the estimate or invoice.
On a case-by-case basis, on approval, for recipients who may have already paid the hospital or clinic, FFF may issue the payment directly to the recipient upon receiving proof of full payment from the hospital or clinic. A direct mailing address will be requested upon approval of the reimbursement and confirmation of full payment.
The recipient agrees to provide any documentation needed for FFF accounting or tax purposes. The medical provider must accept payments over the phone and be able to supply a receipt or invoice directly to FFF. If an insurance reimbursement is greater than the amount provided, the recipient will return the different to FFF.
All recipients of any FFF funds will provide photos, videos, or updates upon requests that may be used on, but not limited to, FFF social media accounts, finleyfortfoundation.org, or other marketing materials.
