If you are an individual in advanced mental health recovery seeking support for any aspect of your employment needs and are able to independently organize and meet with our team, please complete this form! Clinicians, parents, and other allies should not complete this form - they should email info@coleresourcecenter.org to find suitable resources for their clients and loved ones.

Thank you in advance for completing this survey. It has been designed so that CRC can best understand and meet the varied needs of our Workforce community. It should take no more than 5 minutes to complete and will allow us to meet with you to discuss our services. There are 4 sections of questions: Basic Info, Workforce Needs, Wellbeing, and Demographics (military service, cultural/ethnic background, and gender/sexuality). You may elect to skip any or all of the demographic section by selecting "Prefer not to say." Please note that we do not ask or require information about mental health conditions; by completing this survey, we assume that you meet the eligibility criteria of "living with a mental health challenge" and are seeking our specific services for that reason. Please note that in order to meet with CRC you must have access to a device and stable connection for Zoom meetings and that, regrettably, at this time, CRC is only able to offer services in English.
