Sprout Camp Dates and hours -

June 16- July 24, Mondays-Thursdays, 9am-3:30pm (half day and full day options) Camp will not be in session on June 19th in observance of Juneteenth.

Discounts

Multiple Session Discount

$15 off each additional single camper session

Sibling Discount - 10% discount per regularly priced session
This discount applies to campers in the same household and will be applied at billing.

Cancellations are subject to a fee of $75.
To protect Camp Staff, no refunds will be issued two weeks prior to the event.

This form is only for registering one camper.
If you need to Register additional Campers, select which camper you are registering with each new form. This will determine your pricing when you check out.

PHOTOGRAPHY CONSENT FORM/RELEASE
Parent/Guardian First Name* Parent/Guardian Last Name* hereby grant permission to MHC and its representatives to take and use: photographs and/or digital images of me and/or my child for use at MHC activities, in news releases, Facebook® pages related to MHC and youth ministries, PowerPoint slide presentations, and/or promotional materials. These materials might include printed or electronic publications, websites, or other electronic communications. I further agree that my child’s name and identity may be revealed in descriptive text or commentary in connection with the image(s). I authorize the use of these images without compensation to me. All negatives, prints, and digital reproductions shall be the property of MHC representatives. I also understand that the Camper may be photographed or appear in video for such purposes as MHC deems necessary.
