*This release form is a contract with legal consequences and applies to all OUTLoud Inc. Marathon County LGBTQ+ Center ("OUTLOUD INC") events and programs.

Acknowledgement of Risks: I acknowledge the risks associated with this activity and any other risks as presented to me by OUTLOUD INC, its leaders, and its partner organizations in verbal or written form.

Safety Guidelines: I agree to follow all the safety guidelines and emergency procedures of OUTLOUD INC and its partner organizations. I understand that harassment of any kind is not tolerated by OUTLOUD INC and I recognize that any behavior intended to create discord, discomfort, or restricting volunteers or leaders from participating will not be tolerated. I attest that I am physically fit and prepared to perform the tasks offered to me as an OUTLOUD INC volunteer. I further agree to use my personal insurance as the primary provider in the event of injury due to my work or participation as a volunteer for OUTLOUD INC. I shall not operate a personal vehicle for volunteer activities unless I have at least the minimum amount of liability insurance required by Wisconsin law. Finally, I will report possible hazards or unsafe activities to OUTLOUD INC leaders and the leaders of its partner organizations.

Express Assumption of Risks and Responsibility: I assume full responsibility for all the risks associated with my participation. My participation is purely voluntary. I understand the requirements and risks of this activity/program/event and have had the opportunity to ask OUTLOUD INC and its partner organizations for clarification about these requirements and risks. I assume full responsibility for myself and any of my minor children for whom I am responsible, for any bodily injuries, accident, illness, paralysis, death, loss of personal property and expenses thereof, as a result of any accident which may occur as a result of my participation in events or programs.

Loss of Volunteer Personal Property: I hereby release OUTLOUD INC, its leaders, and its partner organizations in the location this event takes place from
liability for any loss or damage of personal property while participating in this event.

Event and Program Release: I hereby release OUTLOUD INC and its partner organizations from any and all liability of any nature for any and all injury or
damage, as a result of my participation in this volunteer event.
