Instructions: The report is not to be evaluative or supervisory. It should reflect the time spent with the local pastor and areas of growth. There should be no surprises. The local pastor will receive a copy of this report and asked to sign it. Then, the report will be sent to the mentor, the local pastor, and the Center for Clergy Excellence.
