Transfer of Ownership
1. This document serves as proof of ownership until such time that a breeder forwards the signed pedigree.
2. Transfer of ownership must be completed and signed by the breeder upon handover of the dog.
3. In the case of transfer to more than one owner, current owner should indicate other owner(s) name(s) here, and submit a form for each owner, on the same date. Failure to do so will cause the dog to be transferred only to one owner.
4. Note: Unilateral Cryptochirid = one, Bilateral Cryptochirid = no, Entire = both testicles have descended in the scrotum.
5. Full regulations with regard to transfer of ownership can be found in the Breed Ordinance, Section 6.
