2026 Community Service Award Submission

This form is not required. This is an optional application for the Community Service Award. A separate panel of judges will determine the winners by reviewing this submission along with your Interview Fact Sheet and Community Service Initiative (CSI) Statement. The winners are not chosen solely on the $ amount raised nor the total # of community service hours. The winners are chosen based on the quality and totality of all aspects and information on the Track Sheet, Fact Sheet, and CSI Statement. Due on/before February 8, 2026 at 8pm.
