Waiver and Release of Liability
I, the undersigned participant, acknowledge that participation in the Black Women Hike involves physical activity and inherent risks, including but not limited to falls, weather conditions, terrain hazards, and encounters with wildlife. I understand these risks and voluntarily assume full responsibility for any injuries, damages, or losses that may occur as a result of my participation.
I release and hold harmless Black Wellness Club, its founders, board members, volunteers, and representatives from any liability, claims, or demands arising from my participation, including claims of negligence.
I certify that I am physically fit to participate in this hike and have disclosed any medical conditions that may affect my ability to safely participate. I agree to follow all event rules and instructions from organizers.
Media/Photo Release
I grant permission to Black Wellness Club and its affiliates to take photographs or videos of me during this event. I understand that these images may be used for promotional, educational, or social media purposes. I release Black Wellness Club and its representatives from any claims related to the use of such images.
By signing below, I acknowledge that I have read, understood, and voluntarily agree to this waiver, release, and media consent.
If participant is under 18:
I am the parent/legal guardian of the participant named above and consent to their participation. I also grant permission for photographs or videos of my child to be used as described above. I release and hold harmless Black Wellness Club and its representatives as described above.
