  • Release of Liability

    For and in consideration of the opportunity to participate in this 2025 Jubilee Canonization Pilgrimage involving travel outside the United State of America to Rome and Assisi Italy I being under no legal disability, on my behalf, as well as on behalf of my heirs, executors, administrators and assigns do hereby release the Diocese of Grand Island, the Most Reverend Joseph Hanefeldt and his successors in Office, diocesan staff, parish staff, and adult volunteer leaders from any liability, cause of action, demand for damages, expenses, compensation and claim on account of or in any way growing out of personal injuries, death, or property damage which may result from my participation in travel abroad. I further expressly waive my right to bring legal action of any kind for any of the claims released.

    I realize that travel outside the United States of America involves risk of personal injury, death, or property damage. I understand that certain risks are inherent in foreign travel and that I fully accept those risks which may include, but are not limited to, war, quarantine, civil unrest, disobedience or terrorism, public health risks, exposure to communicable diseases, criminal activity such as assault, kidnapping and theft, adverse weather conditions and natural disasters such as earthquakes, windstorms and floods, animal attack; insect and animal bites, ill effects of unfamiliar food and water, incidents related to ground, air, or water transportation to include motor vehicle collisions and plane crashes, injuries or damage to property, and other physical, mental, and emotional injury. I fully assume personal responsibility for the consequences of the enumerated risks, including the risk of catastrophic injury or death, and all other potential hazards which may arise in connection with my travel abroad, which may result in injury, death, or damage to property. I hereby assume full responsibility for learning of, assessing, and minimizing all dangers of foreign travel. In signing this release, I have not relied on any statement or representation of the Diocese of Grand Island, its employees, officers, or agents, regarding the nature of any risk, chance, or hazard to the safety of my person, or property which may arise in connection with my participation in foreign travel. This release specifically includes but is not limited to a release of the Diocese of Grand Island, The Most Reverend Joseph Hanefeldt and his successors in Office, diocesan staff, parish staff, and adult volunteer leaders for negligent conduct which may result in personal injury, death, or property damage. I understand that Iwill be personally and solely responsible for providing any insurance which I deem necessary for my protection. I understand that there will be optional activities that are outside of the Jublilee Canonization Pilgrimage which are completely at my own risk.

    Participants are strongly encouraged to consult the State Department Consular Information Sheets and Travel Warnings at www.travel.state.gov/travel  and the Centers for Disease Control (CDC) at www.cdc.gov with regard to their destination country prior to signing this Agreement.

    I also grant permission to the Diocese of Grand Island, their staff, parish staff, and parish volunteer leaders to use photo and video of me/my minor child for publicity/marketing purposes.

  • Emergency Medical Consent Form

    In the event of a medical emergency, I hereby grant the Diocese of Grand Island, their staff, and volunteer adult leaders permission to take whatever action they feel is warranted under the circumstances regarding the health and safety of me. I give authorization to the attending physician, dentist, or medical personnel for any required immediate treatment at the time of the emergency. I agree to be financially responsible for any and all medical expenses and/or treatment costs and all related services provided and I release the Diocese of Grand Island, their staff, and volunteers from any liability.

  • Code of Conduct

    ·   Be Respectful and Safe. The use of our bodies, language, and humor must always honor the dignity of every person.

    ·   Stay in a group at all times unless the leader informs you otherwise and always travel in pairs. 

    ·   Support one another. lf conflict arises or you witness someone in our pilgrimage group acting inappropriately, please do your best to diffuse the situation and then notify a leader.

    ·   Be aware and promote individual and group safety at all times. This is extremely important and cannot be stressed enough. Be sensitive to heat-related headaches and dehydration symptoms.

    ·   Respect lodging facilities. Room occupants are responsible for the condition of their rooms. Damage to them, the building, furniture, or equipment will be charged to those responsible.

    ·   Rest is important during this high energy, busy excursion. Be respectful of others who may need more rest.

    ·   No visitors from other pilgrimage groups are allowed in your hotel room.

    ·   Respect each other's privacy. Males and females will be housed separately and are not allowed in each other's rooms.

    ·   Participants are responsible to check with their cell service regarding international use. The Diocese will not be responsible for loss or damage to any personal property.

    ·   Purchase, possession, or consumption of illegal drugs at any time during this pilgrimage is prohibited. Any violation will result in the participant being dismissed from the diocesan pilgrimage and accomodations immediately. In addition, they may be held accountable by law enforcement for any legal consequence determined by law. 

    ·   Participants are to follow the rules of the local law within the country in which we are traveling regarding the legal age for the consumption of alcohol. The misuse or abuse (i.e. intoxication) by alcohol will be prohibited. Any violation will result in the participant being dismissed from the diocesan pilgrimage and accomodations immediately.

    ·   Purchase or possession of any weapon is prohibited.

    Dress Code 
    In order to promote a safe, healthy, and positive community experience the following dress code must be followed by all male and female participants. Clothing should be modest and appropriate for the setting of a religious conference. Clothing should not present messages contrary to the values the Catholic Church.

    • All attire should be sized to fit without exposing undergarments, buttocks, stomachs, or cleavage. 
    • Shirts must be long enough to naturally touch the top of the lower garment, with arms raised.  Shirts must cover the shoulders. No Spaghetti straps, tank tops, and halter tops or transparent/mesh clothing or shirts with revealing necklines or armholes.
    • Shorts and skirts must be fingertip length when arms are resting at sides.
    • No yoga pants or form-fitting pants/shorts.
    • No clothing or accessories that promote drugs, alcohol, or tobacco either by brand or message.
    • No clothing or accessories with sexually suggestive language or messages or clothing or accessories that promote violence, criminal activity, intimidation, or intolerance of others (based on religion, ethnicity, gender or lifestyle).
    • Modest swimsuit appropriate for a Catholic retreat setting, No Speedos or bikinis.

    Upon submission of the Grand Island Diocese registration form, you will receive a second registration form for ALP (ALTUM L'ALTO PILGRIMAGES) the travel group we are working with in Italy which provides our transportation, lodging, and some meals. If you have a current passport, please complete this form immediately. If you waiting for your passport, you will need to complete the form once it is received.
