Each rider must sign this form prior to event participation.

In accordance with the spirit of volunteerism and service, I assume full and complete responsibility for any injury or accident that may occur to me during my participation in this voluntary event, or while I am a voluntary participant on this rideshare from Galveston, Texas, to Houston, Texas. I hereby release and hold harmless United Way of Galveston, other participants, and all sponsors, persons, and entities associated with United Way of Galveston from liability for injuries and damages sustained to me, whether caused by negligence of the event organizers, sponsors, other persons, or entities associated with this event, or otherwise.

I also grant United Way of Galveston permission to use any photo or video footage that may contain my image captured during the course of the trip in conjunction with any of their publicity programs.
