  • Task Force Animal Hospital (TFAH) provides veterinary services to individuals and families with low income or those receiving social assistance. You will need to provide an income proof for EACH family member related by blood, marriage, adoption or common-law who is 18 years or older living at the same address.

  • Income + Eligibility Verification

  • To qualify for services through Task Force Animal Hospital (TFAH), proof of eligibility is required prior to your first appointment.

    Please provide one of the following for each adult (18 years or older) living in the household who is related by blood, marriage, adoption, or common-law:

    • Proof of government assistance (such as AISH or Alberta Income Support), including a photo or copy of your most recent deposit notice; or
    • A copy of the Canada Revenue Agency (CRA) Notice of Assessment (NOA) or Re-Assessment for the most recent tax year available.

    Annual Income Verification

    Eligibility must be verified using the most recent available tax year.

    • Until June 30, we may accept the previous year's Notice of Assessment if the current year's assessment is not yet available.
    • After June 30, the current tax year's Notice of Assessment is required.
    • Documentation for AISH or Alberta Income Support will generally remain valid until the next annual renewal period.
    • A Notice of Assessment may only be used once for eligibility. Future applications will require a newer year's assessment.
