If selected, you will be asked for documentation to support the data you provide. Likewise, all correspondence and award agreements will be generated from the personal information submitted below. So, please use proper spelling, capitalization and punctuation.

Note: To receive scholarship funds, recipients must be a member of South Bay Credit Union with an open and active share account. If the recipient is not already a member at the time of award notification, they will be required to establish membership and open a share account prior to disbursement of funds. Failure to establish membership prior to the check presentation will result in forfeiture of the scholarship.

Individuals who have previously received this scholarship are not eligible to apply.
