    • "Hillside Handyman" is a ministry of Hillside Community Church located in Rancho Cucamonga, California.
    • Each Service Request is evaluated to determine if the project request can be pursued by the Hillside Handyman team.
    • Hillside Handyman provide labor at no cost.
    • Hillside Handyman can only provide services for owner-occupied residential properties. Commercial properties are not eligible.

    Description of Activity:  Home repair and "fix-it" projects carried out by the members of the Handyman's Ministry Serve Team

    Date(s):  April 5, 2025, July 12, 2025 and November 1, 2025 and various future dates.

    Location of Activities:  Rancho Cucamonga, Upland, Fontana, Ontario and surrounding areas.

    PARTICIPANT AGREEMENT:

    I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING AND/OR VOLUNTEERING IN THIS ACTIVITY OR EVENT, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.

    I certify that I am physically fit, have sufficiently prepared or trained for participation in the activity or event, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity or event.

    I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity or event in which I may participate, and that it will govern my actions and responsibilities at said activity or event.

    In consideration of my application and permitting me to participate in this event, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

     (A)    I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from these events, the following entities or persons: Hillside Community Church and/or their directors, officers, employees, volunteers, representatives, and agents, the service recipients/homeowners, the activity or event holders, activity or event sponsors, activity or event volunteers;

    (B)   I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this document from any and all liabilities or claims made as a result of participation in this activity or event, whether caused by the negligence of release or otherwise.

    I acknowledge that the Hillside Community Church and their directors, officers, volunteers, representatives, pastors, staff and agents are NOT responsible for the errors, omissions, acts, or failures to act of any party or entity conducting a specific event or activity on behalf of the Hillside Community Church.

    I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity or event. Further, I understand that efforts will be made to contact my emergency contact but I give permission to the activity leader necessary for treatment. Should there be no activity leader available, I give permission to the attending physician to treat me.

    I understand that at this event or related activities, I may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by the event holders, producers, sponsors, organizers, and assigns.

    The accident waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

    I certify that I have read this document, and I fully understand its content. I am aware that this is a release of liability and I sign it of my own free will.

  • Hillside Handyman Code of Conduct

    All Hillside Handyman Serve Team Members are asked to agree to adhere to a high standard of ethical, moral and professional behavior including but not limited to the following:

    • Minimum age to serve with Hillside Handyman is 18 years old.
    • Always work in teams of two (2) or more. Never leave a job-site worker alone on-site (even for a quick trip to the store).
    • Never return to a home unaccompanied after a job is completed.
    • Follow-up work must be authorized and scheduled by the Hillside Handyman Ministry Leadership.
    • We serve together by displaying our love for Jesus and also represent Hillside Community Church and those we serve.
    • Honor God in all things - workmanship, language, dress, manners, professionalism.
    • Pray for the needs and concerns with the homeowner - do not attempt counseling, rather discuss situation with your Team Lead.
    • Do not solicit business, hand out business cards, or phone numbers
    • No alcohol, drugs, profanity or smoking while serving with the Hillside Handyman or on the Project Site.
    • No radios or loud music on the job.
    • While serving, or on Project Site, ensure you are dressed modestly (do not remove shirt while working).
    • Do not add to the scope of the project work. If homeowner is requesting additional work, refer them to the next work date or to your Team Lead.
    • Leave the property in better shape than you found it.
    • Clean up after yourself.
    • Do not accept or receive any form of payment for service under any circumstances. Refer homeowner to Team Leader - we serve God alone.

  • Hillside Handyman Ministry

    Hillside Handyman Ministry members desire to use our gifts and time to help with household "fix-it" project that you are unable to do on your own. If you are able to pay for materials used, we will gladly donate our time and labor to serve. If you are unable to pay for the materials, please let us know for alternate consideration.

  • Service Recipient Indemnification, Waiver and Release of Liability

    “Service Recipient" listed above whose residence address is listed above as the “Service Recipient’s Residence” acknowledges that the Handyman’s Ministry, a volunteer ministry coordinated by the Serve Ministry and Men’s Ministry of Hillside Community Church, a California non-profit religious corporation (individually and collectively “Hillside”), desires to use its gifts of volunteer time and labor to help with certain household projects (individually and collectively, the “Project”) at the reasonable request of Service Recipient at Service Recipient’s Residence, which Service Recipient has been unable to complete, and upon the prior approval by Hillside of such reasonable request. 

    Service Recipient shall supply, at their own cost and expense, all materials concerning the Project; provided, that if Service Recipient is unable to pay for such materials, Service Recipient shall so notify Hillside prior to the commencement of any Project, and Service Recipient acknowledges that, in such case, Hillside shall use reasonable ministry efforts to assist Service Recipient to obtain materials for the Project, but with no assurance that such materials will be so obtained by Hillside.  

    Approval of Service Recipient’s reasonable request for the Project shall be in Hillside’s sole and unilateral discretion, and Hillside reserves the right to revoke its approval of Service Recipient’s reasonable request at any time, for any reason or for no reason, whether or not the Project is partially or fully completed.

    Service Recipient recognizes that Hillside has not undertaken any duty or responsibility for his or her safety and assumes full responsibility for all risk of bodily injury, death, disability and property damage concerning Service Recipient, Service Recipient’s Residence and the Project.  Service Recipient recognizes that these risks include, but are not limited to, physical injury to Service Recipient and to third parties.

    Service Recipient and Service Recipient’s heirs, executors and administrators hereby indemnifies, defends, covenants not to sue and holds Hillside and each and every past, present and future affiliate, partner, servant, director, officer, employee, representative, contractor, vendor, volunteer, donor, attorney and agent of Hillside harmless from and against any and all claims, damages, injuries, causes of action, suits, debts, liens, contracts, judgments, obligations, agreements, promises, liabilities, demands, losses, costs, attorneys’ fees and court costs or expenses of any kind, character or nature whatsoever, from or related to the Project or Service Recipient's participation in the Project, including, but not limited to, claims for damages arising out of, incidental to or concerning third party claims.

    Service Recipient and Service Recipient’s heirs, executors and administrators hereby waives, releases and forever discharges Hillside and each and every past, present and future affiliate, partner, servant, director, officer, employee, representative, contractor, vendor, volunteer, donor, attorney and agent of Hillside from any, every and all claims, damages, injuries, causes of action, suits, debts, liens, contracts, judgments, obligations, agreements, promises, liabilities, demands, losses, costs, attorneys’ fees and court costs or expenses of any kind, character or nature whatsoever, known or unknown, fixed or contingent, which Service Recipient has, may now have, or may hereafter have against said persons, or any of them, by reason of any matter, cause, or thing whatsoever arising out of or based upon any action or omission of or concerning the Project or Service Recipient's participation in the Project, or any of them.

    Service Recipient acknowledges that Hillside has made no representation or guarantee to Service Recipient concerning the quality or timing of the work to be performed as part of the Project; that Hillside has made no warranty of any kind, character or nature whatsoever, and disclaims any warranty, express or implied, including, but not limited to, the implied warranty of merchantability and fitness for a particular purpose regarding the Project; is not relying on any oral representations, statements or inducements from Hillside; and that Service Recipient’s Residence address may be used by Hillside for notice purposes.

    THE ABOVE IS ACCEPTED BY SERVICE RECIPIENT ON THE DATE SET FORTH BELOW.
