Mustangs Academy Application Form

What is it?

A strategic partnership with The Coaches Assistant to deliver high-quality group-based football-specific strength & conditioning, speed and agility for current and emerging Mustangs players aged 15+.

This will be limited to a minimum of 12 - max 24 athletes due to the capacity.

What will it look like?

Training Structure of 12 weeks starting from week of March 17 - week of June 2, consisting of;

1x 60 min training session on the field per week @ Vikings Rugby Club, Wollongong Monday Nights 7:30 pm
- 1x 60 min gym session per week @ Rekreate Gym 15/2 Drummond Street, Coniston Wednesday Nights 6:30pm

This will include:

Group-based assessments (aerobic/anaerobic capacity, maximal strength)
Remote Programming for athletes focusing on strength, conditioning, agility and injury prevention

The total cost will be $125 per athlete (approx $10p/w)

Are there any injuries (lingering or major) that need to be taken into consideration during the training sessions by the coaches?*

In consideration of myself/my child being allowed to participate in any way in Mustang Academy’s programs, related events and activities, I/the Parent/Guardian acknowledge, appreciates, and agrees that the risks of injury and illness (ex: communicable diseases such as MRSA, influenza, and COVID-19) to myself/the Child from the activities involved in these programs are significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce these risks, the risks of serious injury and illness do exist; and Parent/Guardian agree as follows:

I/PARENT/GUARDIAN AND CHILD KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES (as defined herein) or others, and assume full responsibility for my/the Child’s participation;

I/PARENT/GUARDIAN AND CHILD knowingly and willingly agree to comply with and agree to the Gridiron Football Terms of Service and Conditions. If I/the Parent/Guardian observe any unusual significant concern in my/the Child’s readiness for participation and/or in the program itself, I/Parent/Guardian will remove myself/the Child from participation and bring such attention to the nearest Academy official immediately;

I/PARENT/GUARDIAN AND CHILD KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS of myself/the Child contracting and/or carrying a communicable disease such as MRSA, influenza, and COVID-19 and that the risks associated with a communicable disease are dangerous, including the potential for permanent disability and death. While particular rules, equipment, and personal discipline may reduce these risks, the risks of serious illness exist;

I/PARENT/GUARDIAN AND CHILD and on behalf of my/their heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Mustangs Academy; its directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“Releasees”), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property incident to the Child’s involvement or participation in Gridiron programs, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law;

I/PARENT/GUARDIAN AND CHILD on behalf of my/their heirs, assigns, personal representatives and next of kin, HEREBY INDEMNIFY AND HOLD HARMLESS all the above releases from any and all liabilities incident to the Child’s involvement or participation in these programs, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law. I/Parents/Guardians and Child promise not to sue Mustangs Academy nor its directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct events; and

The Parent/Guardian asserts that I have explained to my Child the risks of the activity and his/her responsibilities for adhering to the rules and regulations and that my Child understands this agreement.

I/PARENT/GUARDIAN AND CHILD, will allow Wollongong Mustangs the exclusive right to utilise content such as photos, videos, interviews, sound recordings, etc, in pre-, post, during, and future purposes. Including but not limited to: name, image, and likeness of athletes, coaches, and spectators.
