How to Report an Incident Appropriately
For an Incident Report to be most effective, it should be clear, factual, and objective. The report may be used for investigation, disciplinary review, or tribunal hearings, so it is important to provide accurate and detailed information.
Best Practices for Reporting an Incident:
- Stick to the Facts
- Describe exactly what happened without personal opinions or assumptions.
- Avoid exaggeration or speculation, only include what you saw or heard.
- Be Specific and Detailed
- Include who was involved, what occurred, when, and where the incident happened.
- If applicable, mention any actions taken immediately after the incident.
- Use Clear and Concise Language
- Write in complete sentences and use neutral, professional language.
- Avoid emotional or vague statements, state what happened as clearly as possible.
- Include Witnesses (if any)
- If there were other people who saw or heard the incident, include their names and roles (e.g., referee, coach, spectator).
- Witness accounts can provide additional perspectives and support the report's accuracy.
- Submit as Soon as Possible
- Complete and submit the report immediately or as soon as possible after the incident to ensure accuracy and reliability.

Items to include (but not limited to):
- Names/Jersey Numbers.
- Location.
- Game time and date.
- Team Names.
- Positions (coach, player, spectator, referee, etc.).
- Detailed report of what happened:
- What was said.
- What actions occurred.
- Etc.
- Any footage, recordings, etc.
- Other witnesses that can attest to your version of events.
For Online Incidents:
- Screenshots of messages.
- Saved voice recordings.
- Etc.
The overall aim of wirting an incident report is to give CBBA an opportunity to investigate these incidents thoroughly and fairly. Any report that lacks critical information will not be used.
