We, the undersigned, hereby grant the Atlanta Archdiocese Haitian Chaplaincy & Rendez-Vous Lanmou permission to photograph and use our likeness in the form of photographs, videos, or other digital media (the “Images”) for the purposes of marketing, advertising, and other promotional activities. We understand that the Images may appear on the company website, social media platforms, printed materials, or other marketing avenues.

By signing this waiver, we affirm the following:

1. Consent to Use: We consent to the use of our Images for the purposes described above.
2. No Financial Compensation: We understand that no financial compensation is being provided for the use of the Images, unless otherwise stated.
3. Revocation: We may request that the Images be removed from public use by notifying the Atlanta Archdiocese Haitian Chaplaincy & Rendez-Vous Lanmou in writing.

By signing below, we acknowledge that we have read and understood this Photo Release Waiver, and we agree to the terms set forth.
