We will cover the cost of rabies vaccination if your dog or cat is not current. If your animal is current, please provide proof of rabies vaccination when you bring them in for their appointment.

You can come back to this form and fill out the remainder once you know which vet you will be using. 

PLEASE DO NOT GO TO THE VET WITHOUT A VOUCHER FIRST. Calling us while you are at the vet and expecting us to generate an immediate voucher is not an acceptable way to receive a voucher from us. Please fill out the form and wait for your voucher to arrive before making your appointment. Please give us at least 48 hours to get the voucher ready for you.
