1. The patient must reside within Kendall County.
2. Complete and submit the required hospice registration form. Hard copies of this form can also be found at https://hospice.kendallcoroner.org.
3. The physician listed on the hospice registration form MUST be willing to sign the death certificate according to their diagnosis.
4. When death occurs, please advise the responding medical professional (LPN, RN, APN, or NP) to call the Kendall County Coroner's Office directly at (630)553-4200. If after hours, please follow the appropriate prompts to have an on-call investigator paged.
5. The on-call investigator will verify that the registration information is accurate at the time of the call, as well as ask for the time of death, recent falls/injuries, and recent vaccinations. All information on the registration form should be available for the hospice representative to relay to the investigator at the time of notification. The hospice representative must receive permission BEFORE releasing the body to the funeral home.
6. If the patient moves, dies outside of Kendall County, or is no longer receiving care from your hospice agency, please notify the Kendall Coroner's Office immediately at staff@kendallcoroner.org or (630)553-4200.
7. At the time of death, the Kendall County Coroner maintains jurisdiction over the decedent and can make any changes to this protocol that are deemed necessary (including a formal on-scene investigation involving police, fire, or a Coroner's Office representative).
