By completing this form, you are accepting responsibly for the purchase of tools on Mostlane Construction credit account in-line with the agreement set out within this contract:

- Approval must be received following completion of this form prior to any purchases being made.

- Any purchases made without approval will be charged at full price inc.VAT and 20% markup. 

- You are responsible for ensuring the total price of the tools is accurate as below and you will be responsible for any additional charges or incorrectly priced items.

- These tools/plant will remain the property of Mostlane Construction until paid in full.

- Purchases of tools or plant are only approved following completion of this form and email confirmation of approval via the email address you supply below.

- You agree for monies to be deducted from your weekly pay until the balance has been cleared in accordance with this agreement as set out below.

- Repayments start from next pay due following completion of this form subject to approval.

This agreement does not affect your statutory employee rights under the Employment Rights Act 1996
