TATTOO CONSENT POLICY

1. Consent Declaration

By submitting this form, I acknowledge that I am aware that getting a tattoo is a permanent procedure and may alter the appearance of my skin and body in an irreversible way.

2. appointments deposit

I understand that the appointment deposit is a non-refundable amount.

3. Use of Images

I authorize my tattoo to be photographed and/or recorded for the artist’s portfolio and promotional purposes. If I wish to revoke this authorization, I may do so at any time by providing written notice.

4. Responsibilities

I understand that the artist is not responsible for allergic reactions, infections, or any complications resulting from the tattoo, especially if I fail to follow the provided aftercare instructions properly.

5. Liability Waiver

I acknowledge that Malik Zamorano cannot be held liable for any costs, damages, or legal implications arising from the tattoo procedure.

6. Age

I confirm that I am over 18 years old and that all the information provided is true.
