    CAS Assessor Certification Form

  • Welcome to the CAS Assessor Certification Form! To get certified and access the new CAS agency in our community's Homeless Management Information System (HMIS), where all CAS Assessments are available, users must complete the following steps:

    Note: If you do not have a HMIS account, please request access here before moving onto the following steps.

    • Attend or View Training: Visit our CAS Assessor Certification Training webpage to attend or watch the CAS Assessor Training.

    • Complete the Certification Form: Fill out and submit the CAS Assessor Certification Form.

    • Sign the CAS Assessor Agreement: After submitting the form, you’ll receive an email request to e-sign the CAS Assessor User Agreement. Check your spam/junk folders for emails from Jotform.

    • Complete the CAS Assessor Quiz: Once you’ve signed the CAS Assessor User Agreement, the CAS Assessor Quiz will be sent to you via email. There are 31 total questions. Please plan for 60 minutes to complete the quiz. You will have 3 available attempts for a 100% passing score and 90 days to complete these 4 steps.

    After passing the quiz, you’ll gain access to the Coordinated Access System (CAS) agency in HMIS through your current HMIS account. Please allow 3 days for the CAS agency to display in your portal.
