St Andrews Lutheran College has engaged us to review your immunisation history, as part of their commitment to creating a safe and healthy work environment.
The goal is to ensure that all staff are up to date with their recommended vaccinations. The information you provide will allow us to assess your vaccination status and determine if any additional vaccines are needed.
In the form we will ask for your personal details, including your full name, date of birth, and contact information, to help identify you and maintain accurate records.
We also ask for your Medicare number (or Individual Healthcare Identifier [IHI] if you don't have a Medicare number), which we use to review any vaccination records that are recorded by medicare.
You will also be able to upload any vaccination records you may have to show you're immune to particular diseases.
Finally, the form will ask for your consent to review your immunisation history so we can provide any necessary updates or recommendations, as well as share this information with your employer.
