2025 State Team Nomination Form - Management (Team Manager & Head of Delegation)

Prior to completing this nomination form, please ensure that you have read all the informaiton available regarding the 2025 Australian Championships State Team.

Please note that not all indicated positions may be selected. The number of available positions on each Team will be determined with consideration to the Team composition.

The State Team t-shirt is different from the WA Team uniform and is a t-shirt all successful athletes receive to signify the team experience. Managers are able to purchase these t-shirts at their own cost ($32), however they are not compulsory. 

If you would like to purchase a t-shirt, please select your preferred size according to the below sizing chart. 

Please use the measurements to select your size to ensure the t-shirt will fit.

All successful team members will be required to complete medical forms distributed after selection and returned prior to travel. 

To assist the selection committee, please outline some basic medical information below. 

2025 State Team Procedure

All nominees should be familiar with the content of the 2025 State Team Procedure, Member Protection Policy and Social Media Policy. Please ensure you read and understand these policies/procedures. 

They can all be found on the relevant State Team pages on the Gymnastics WA website as well as the Procedures & Policies page. 

State Team Declaration

All nominees are required to agree to the below declaration in order to successfully nominate for a position on the 2025 State Team.

 

"I wish to be considered for a place on the 2025 Gymnastics WA State Team to attend the 2025 Australian Gymnastics Championships and agree to the following State Team declaration. 

I understand that the selection in the WA State Team is based on meeting the selection criteria as specified in the Gymnastics WA 2025 State Team Procedure. 

If selected in the team, I agree to wear all team uniform items in accordance with team regulations.  

I understand that selection on the team is dependent on the agreement to abide by the Gymnastics WA Member Protection Policy and that breaches of the Code of Conduct may result in disciplinary action. 

I understand that once selected, I will need to comply with the terms and regulations outlined in the State Team Procedure. 

Should I need to withdraw from the team, I must notify Gymnastics WA immediately and submit a State Team withdrawal (accompanied by a Medical Certificate if relevant). I understand that if I withdraw from the team, that I will not receive any funding if relevant to my position. 

I understand that, if applicable, I am responsible for paying the State Team invoice issued by Gymnastics WA for relevant costs associated with the State Team. I am aware that if I do not pay this invoice by the specified due date, that I will not be permitted to travel to Australian Championships.

I agree to abide by the event Integrity Education requirements including, but not limied to, completing the Sports Integrity Australia (SIA) Drugs in Sport  online course requirements as well as the GA event specific online courses. 

I agree that Gymnastics WA or any team member associated with the 2025 Australian Gymnastics Championships shall not be deemed responsible or liable in any way for any injury, illness or other mishap that may occur to me during the tour. I agree to be responsible for the costs of any medical treatment and ambulance deemed necessary by the Event Doctor and/or Head of Delegation. 

I have read and completed the relevant questions in this nomination form and the specific Gymnastics WA Procedures and Policies and understand that any breach of the rules could lead to me being ineligible for selection.”

Please note that if you are a parent of an athlete nominating for a Management position, but your athlete is unsuccessful in being selected onto the team, then the selection committee will not consider you for the management role.
