By signing below, each of the above listed business and business owners (individually and collectively, "Applicant") certify that all information provided in the application is true and accurate. Applicant shall immediately notify J & A Marketing d/b/a Integris Financial Group ("Integris") of any change in such information. Applicant authorizes Integris Financial Group to share this application and such other information as obtained hereunder with each of its representatives, successors, assigns, designees and third-party funding partners, which includes lenders and other finance providers with whom Integris Financial Group has, or may in the future enter into, commercial-brokerage-financing relationships (collectively, "Recipients" Further, Applicant authorizes Recipients: (i) to request and receive any consumer, business and/or investigative reports about or of Applicant, from one or more consumer reporting agencies, including but not limited to TransUnion, Experian, and Equifax; (ii) to request and receive such other information from third-parties in connection with underwriting Applicant including but not limited to credit card processor statements and bank statements; and (iii) to transmit this form, along with any of the foregoing information obtained in connection with this application, to any Recipient and/or third party as provided by law.
