How to pay your vendor fee(s):

You will be sent your fee total and an invoice in a confirmation email 3-5 days after submitting this form. There are two options for vendor payment: 1) Preferred Method: Check made out to the City of Sun Prairie Parks, Recreation & Forestry Department to 2598 West Main Street, Sun Prairie, WI 53590. 2) Pay via credit card IN-PERSON at the Sun Prairie Parks, Recreation & Forestry office at the Westside Community Services Building. We cannot accept credit card payments over the phone. **If cost is an issue, please reach out and we will work with you.
