Roster Acceptance* This field is required.I/We understand that to officially accept our roster spot, I/We must complete this Code of Conduct agreement and pay our Team Fees. If opting for a payment play, I/We agree to have a payment plan agreement in place to officially accept our roster spot.

Participation Eligibility* This field is required.I/We understand that our Code Of Conduct and Team Fee payment must be completed to be eligible for team activities. In cases of ongoing ineligibility, I/We understand the program reserves the right to remove our player from the roster.

Refund Policy* This field is required.I/We understand there is a no-refund policy once we have officially accepted our roster spot. If I/We officially accept our roster spot and then change our decision before the season begins, refunds will only be given if our roster spot can be filled with another player. Requests for refunds must be submitted in writing and will be reviewed on a case-by-case basis. Special situations (e.g., season-ending injury) will be reviewed individually to determine refund eligibility.
