4-Week Subscription Modification Notice Form

𝗦𝗨𝗕𝗦𝗖𝗥𝗜𝗣𝗧𝗜𝗢𝗡 𝗠𝗢𝗗𝗜𝗙𝗜𝗖𝗔𝗧𝗜𝗢𝗡 𝗥𝗘𝗤𝗨𝗘𝗦𝗧

This form serves as your official four-week notice to modify your Study Buddy subscription by reducing the number of scheduled sessions.

𝗜𝗠𝗣𝗢𝗥𝗧𝗔𝗡𝗧 𝗜𝗡𝗙𝗢𝗥𝗠𝗔𝗧𝗜𝗢𝗡 𝗥𝗘𝗚𝗔𝗥𝗗𝗜𝗡𝗚 𝗬𝗢𝗨𝗥 𝗠𝗢𝗗𝗜𝗙𝗜𝗖𝗔𝗧𝗜𝗢𝗡

• 𝗪𝗵𝗮𝘁 𝗜𝘀 𝗮 𝗠𝗼𝗱𝗶𝗳𝗶𝗰𝗮𝘁𝗶𝗼𝗻? A modification allows you to reduce the number of weekly sessions without canceling your entire subscription. For example, you may request to reduce your schedule from three sessions per week to two.

• 𝗙𝗼𝘂𝗿-𝗪𝗲𝗲𝗸 𝗡𝗼𝘁𝗶𝗰𝗲: Your current schedule will remain active for four weeks from the date this form is submitted. Only the sessions you request to remove will be discontinued after the notice period.

• 𝗣𝗮𝘆𝗺𝗲𝗻𝘁𝘀 & 𝗦𝗲𝘀𝘀𝗶𝗼𝗻𝘀: All scheduled sessions and payments will continue as usual throughout the four-week notice period. Your billing will be adjusted once the modification takes effect.

• 𝗥𝗲𝗳𝘂𝗻𝗱𝘀 & 𝗖𝗿𝗲𝗱𝗶𝘁𝘀: Refunds, credits, and prorated adjustments will not be provided for unused sessions during the notice period.

• 𝗨𝗻𝘂𝘀𝗲𝗱 𝗦𝗲𝘀𝘀𝗶𝗼𝗻𝘀: Any scheduled sessions you choose not to attend during the notice period will be forfeited.

• 𝗣𝗮𝘆𝗺𝗲𝗻𝘁 𝗥𝗲𝘀𝗽𝗼𝗻𝘀𝗶𝗯𝗶𝗹𝗶𝘁𝘆: You remain responsible for all payments due during the four-week notice period, regardless of whether you attend your scheduled sessions.

𝗪𝗛𝗔𝗧 𝗛𝗔𝗣𝗣𝗘𝗡𝗦 𝗡𝗘𝗫𝗧?

Once your modification request is submitted, you will receive a confirmation email outlining the effective date of your new schedule, the sessions being discontinued, and your updated billing schedule.

𝗣𝗟𝗘𝗔𝗦𝗘 𝗡𝗢𝗧𝗘: This form does not cancel your entire Study Buddy subscription. Any sessions not included in your modification request will continue as scheduled.
