Art Exhibit Release of Liability Form
Submitter grants permission for Equitas Health to reproduce his/her name, information describing his/her work, representations of his/her work and any other information he/she has provided for the purpose of display, promotion, and publicity either now or in the future. Equitas Health will take all necessary precautions to protect artwork from damage and theft. However, Equitas Health will not be held responsible for any loss or damage to exhibited and/or shipped artwork. Art Submitter agrees to display, pack, transport, and provide for the return of the artwork at his/her own risk/expense. Art Submitters may acquire insurance to cover loss and/or damage.
For more than 30 years, the art community in central Ohio has given generously to support Equitas Health's lifesaving HIV/AIDS prevention, treatment, and support services. This year, Art for Life will give back by compensating artists 10% of the final auction price for their pieces. Artists accepted into the auction will receive an email requesting that they fill out a W9 form for tax purposes. Remaining funds raised from the auction of artwork will go to support Equitas Health's critical mission. If art does not sell, Equitas Health will notify you, and you will have until December 31, 2025 to retrieve your work from our office at 1105 Schrock Rd. After that, Art will remain in care of Equitas Health and possibly displayed in one of our health centers in Ohio.
Please read the following statements acknowledging your understanding. Form submission acts as Art Submitter Acceptance of Terms:
Indemnification. I release Equitas Health, The Columbus Museum of Art, and their representatives, officers, directors, employees, volunteers and agents and the participating exhibition venue from any and all claims, demands, causes of action and judgments, losses, costs and expenses, including but not limited to reasonable attorney’s fees, arising during the transport, storage, and display of my artwork. “Damage” includes but is not limited to: bending, tearing, marking, blemishing, and theft of my artwork
Insurance. I understand that Equitas Health does not insure my donated artwork, and if I prefer my artwork to be insured I must acquire my own insurance.
