We are currently at capacity and cannot register more students. If you are interested in attending, please submit this form to sign up for the waitlist. Should spots open up, we will contact you.

By submitting this form, you will be added to the waitlist – a registration is not guaranteed.

The following form should be completed by individual students AND their parents. We ask for this to be completed no later than Tuesday February 25, 2025. There is no cost per student/advisor/chaperone. Breakfast and lunch will be provided with vegetarian and gluten-free options (all ingredients will be listed to avoid allergies).

By registering you are NOT guaranteed a seat, as we have limited openings, unless you receive a confirmation email on Monday, February 24, 2025. Students will receive an excused absence from their school with PROOF of attendance by the organizing committee.

Admissions into the conference will be made on a first-come first-serve basis. So the earlier you fill this form, the more likely you will be invited to attend.

• 10:30-11:45am - Breakout Sessions
• 11:45-12:30pm - Keynote
• 12:30-1:15pm - Lunch and Exhibits
• 1:15-1:45pm - Conclusion
• 1:45-2pm - Departure

Permission from Parent:

I, the undersigned, hereby give my permission for my child, a minor, to attend the off-campus event hosted by Jakara Movement on February 28, at Yuba College. I understand and acknowledge the nature of the event and the potential risks involved in participation.

I, the undersigned parent/legal guardian, acknowledge that the Jakara MOvement and its staff will take all reasonable precautions to ensure my child's safety during the event. However, I understand that accidents or unforeseen incidents may occur. I agree to absolve and hold harmless Jakara Movement, its officers, agents, employees, and volunteers from any liability, loss, or injury that may occur to my child during this event, including transportation to and from the venue.

I have read, understood, and agree to the terms above. By signing this form, I confirm that my child is in good health and capable of participating in the event.

I am also, the undersigned, hereby give my permission for my child, , a minor, to attend the off-campus event hosted by the Jakara Movement on February 28, 2025, at Yuba College. I acknowledge that by attending the event, my child may be filmed and photographed during the activities.

I understand that the photos and video footage may be used by the Jakara Movement for promotional purposes, including but not limited to social media, websites, brochures, and other media. I consent to the use of my child's image, likeness, and voice in connection with this event and waive any rights to compensation or ownership related to the photos or videos.

I understand that these images and recordings will be used solely for the purpose of promoting the event and activities of the Jakara Movement. If I do not wish for my child to be filmed or photographed, I understand it is my responsibility to inform the event organizers prior to the event.
