Each volunteer (or their parent/legal guardian) must sign this form prior to event participation.
In accordance with the spirit of volunteerism and service, I assume full and complete responsibility for any injury or accident that may occur to me during my participation in this voluntary event, or while I am on the premises of any United Way of Galveston or other participating entity, including Galveston Children’s Museum and the Moody Mansion. I hereby release and hold harmless United Way of Galveston, other participants, and all sponsors, persons, and entities associated with United Way of Galveston and/or Galveston Children’s Museum and/or the Moody Mansion, in Galveston, Texas, from liability for injuries and damages sustained to me, whether caused by negligence of the sponsors, other persons, or entities associated with this event, or otherwise.
I also grant United Way of Galveston permission to use any photo or video footage that may contain my image captured while I am volunteering in conjunction with any of their publicity programs.
