This form indicates that upon acceptance into the showcase, the participating player authorizes The NAB Academy Ltd (NABA) to act on their behalf according to the showcase's best judgment in any emergency requiring medical attention. The player hereby releases, exonerates, and discharges the showcase/ NABA and its employees, agents, partners, or subsidiaries from any and all actions or causes of action, known or unknown, resulting from any player injuries while attending any of our events.
Parents and players signing this form also grant permission to Get Recruited (the showcase) and NABA employees and agents to use the player's image and likeness, via photography and video footage, for promotional purposes related to both the player and the showcase.
