I acknowledge and agree that a 50% non-refundable deposit is required to secure my party date and time. I understand that my reservation is not confirmed until the deposit has been received.

If plans change, my party may be rescheduled to another available date and time, based on availability.

I also give Dance Party AZ and Studio Z permission to use photos and videos taken during the party for promotional and marketing purposes. All party participants will be required to sign a safety waiver and will have the opportunity to opt out of the photo/video release if they do not wish to be photographed or recorded.
