I do hereby voluntarily submit my application for attendance in Miami Shotokan Karate Club's 31st Anniversary Summer Training Camp, to be held July 23-27, 2025 at Miami Shotokan Karate Club, and I hereby acknowledge and recognize that karate training is inherently dangerous and, accordingly, I hereby personally assume all responsibility for any and all damages, injuries or losses that I may sustain or incur while attending or participating in this event and I hereby waive any and all right to assert any claim against the promoters, operators, instructors, or sponsors of said event, individually or otherwise, including but not limited to Jose Ferrand, Miami Shotokan Karate Club, Inc., Hirose Sensei and Kunio Kobayashi, for any injuries I may sustain. I fully understand that any medical treatment given me on site will be of first aid treatment only. I consent that any pictures furnished by me or taken of me in connection with this event can be used for publicity, promotion or television showing and I hereby waive any compensation in regard to thereof.
