Assistant Coaches – Please attach a copy of your current coaching qualification/accreditation.

Letter of Endorsement - Please note a letter from your child's school is required to endorse their participation as a student official.

 Data entered into this online form will be used in accordance with the SSV Privacy Collection Notice, to update SSV's data records, which includes being added to the SSV monthly eNews subscription.
