  • Part A - General Information

  • Thank you for enroling on an Outward Bound Hong Kong (OBHK) course.

    This form is the first step on a journey of discovery. Before completing the form, please make sure:

    1. The right person is completing and signing the form. If the participant is under 18, this form should be completed by a parent or guardian. For anyone over 18 it should be completed by the participant.
    2. The information you provide is accurate and as detailed as possible. We review all of the information to ensure that we are as prepared as possible to serve our participants – and we can only do that if we have the right information at the right time.
    3. If you have any questions, please contact us at info@outwardbound.org.hk or call 2792-4333. We would prefer to take the time to answer your queries before the course starts, have you arrived confident and with everything you need. There is a lot of information available in the links on this form, and our website at www.outwardbound.org.hk.

  • Part C - Medical History

  • Please be fully transparent in providing OBHK with the information requested. Many participants over the years have successfully attended and completed our courses regardless of any medical difficulties they may have had. Having said that, we must be aware of any such conditions for in order to ensure that our programme and our participant make a good fit. The more information we have, the better we can tailor our programme to your needs.

  • Attention

    • OBHK staff will carry and administer prescription medication brought from home with the prescribed dosage.
    • OBHK does not assume responsibility for any adverse reactions that may occur following administration of medication sent from home. It is the parents/guardians or participants' responsibility to bring the sufficient amount of medication for the duration of the course.

  • Authorisation of Medication to be administered by OBHK Staff

    All OBHK staff carry the below medication and will administer age-appropriate medication doses according to manufacturer's recommendations. Please indicate if you consent to these being administered.

  • Part E - Group Personal Accident Insurance Scheme (OPTIONAL)

  • OBHK carries third party liability insurance. In the event of an accident or illness during the course requiring external treatment, government hospitals or clinics will be utilised, unless in the case of minor illness or accidents whereby private clinics may be used. Participants will be required to pay all private clinical costs. Participants can arrange their own personal insurance or join through the scheme offered by Zurich Insurance Company Limited to participants aged between 7 and 65 years.

    Reminder:

    - Please note that no insurance will be effective unless payment is made before course commencement.

    - The premium is non-refundable under any circumstances.

    - Participants who without HKID card requiring treatment at hospitals must pay a deposit prior to receiving treatment. This process requires the use of a credit card which OBHK will not supply.

  • Part F - Declaration

  • Declararion

    - I, the undersigned, declare that the information provided is true and complete. I agree that my (and the applicant’s) personal data may be used by OBHK and its staff for administration, programming and emergencies.

    - I (and the applicant) have read and understood the details outlined in this document and agree to abide by the rules for taking part in the OBHK course, in particular, to abstain from alcohol, tobacco and illegal substances, as well as to participate fully and co-operatively with the staff and others. I (and the applicant) understand(s) and accept(s) that if the rules or instructions are not observed, I (and the applicant) may be dismissed from the course.

    - I (and the applicant) understand that the course is vigorous and demanding. To the best of my knowledge, there are no medical or other reasons why I (and the applicant) should not take part in this course. I (and the applicant) am/is in normal health with no undeclared pre-existing medical or psychological  conditions or allergies.

    - I (and the applicant) further agree that should I (and the applicant) require medical treatment, I (and the applicant) consent for myself/himself/herself to receive whatever treatment deemed necessary and determined by a registered doctor. I (and the applicant) agree to pay for any/all treatment required for doctor and/or hospital consultations.

    - I (and the applicant) agree to allow Outward Bound Hong Kong to use any and all images and/or testimonials taken of or provided by me (and the applicant)  during the course for any and all marketing purposes.

    - I (and the applicant) have read and understood this release and indemnity agreement prior to signing it. I (and the applicant) am/is aware that by signing below, I (and the applicant) am/is affecting my legal rights and liabilities and the legal rights and liabilities of my heirs, next of kin, executors, administrators and assigns in relation to OBHK. I (and the applicant) undertake to abide by the safety directions given by the staff of OBHK during the course.

  • Consent

    - Consent is hereby given for the participant to attend an OBHK course and permission is given for any emergency anaesthesia, surgery, hospitalisation, medication and/ or other treatment which may be determined necessary by a medical professional.

    - I have read the description of OBHK courses within this enrolment form and I understand that the programme can involve physically and mentally strenuous activities in a remote wilderness area potentially far from urban facilities.

    - I have taken the opportunity to read important information which is available to me in relation to this enrolment, including: 

    • Voluntary Assumption of Risk Statement
    • OBHK Safety Statement 
    • Personal Information Collection Statement

    - If I have indicated that I require regular medication I shall bring it with me. If I fail to do so I understand that I may not be permitted to continue the course.

    - The information provided above is a complete and accurate statement of the physical and psychological factors which may affect my participation on an OBHK course, I realise that failure to disclose such information could result in serious harm to me and my fellow participants and I agree to indemnify and waive OBHK of all liability if all relevant information is not disclosed.

  • Signature

    Electronic Transactions Ordinance (Cap. 533) gives electronic records and digital signatures the same legal status as their paper-based counterparts. By signing the box below, I affirm that the information provided above is a complete and accurate statement of the physical and psychological factors which may affect my participation on an OBHK course.
