Credit Authorization Form

I/We authorize the leasing company, its designees or assignees to make any necessary credit inquiries deemed proper in connection with this lease application. I/We authorize and instruct any person or consumer-reporting agency to compile and furnish to the Leasing Company any information that it may have or obtain in response to such credit inquiries and agree that such information, along with the application, shall remain the leasing company property whether or not the lease is approved.

The undersigned represents that all information provided with this Application is true and correct and I/We hereby authorizes First Financial, LLC, and/or its designees, assignees, and/or Lessor/Secured Party to obtain from third parties information it deems necessary to arrive at a decision regarding this Application. By signing below, the undersigned individual(s) as principal of and/or guarantor for the applicant, authorizes Lessor/Secured Party, its designee, assigns or potential assigns, to review his/her personal credit profile provided by a national credit bureau in considering this Application and for the purpose of update, renewal, or extension of credit to the Applicant or the collection of any resultant accounts. I authorize all deposit, borrowing, financial and trade information to be released to Lessor/Secured Party by telephone, fax and/or email. A photocopy, fax or electronic signature (via DocuSign) of this authorization shall be valid as the original.
